How to Register

Step 1 - Check that you live withing the practice area. If you are unsure then please contact the practice for confirmation.

Step 2 - Call into practice and pick up a registration pack OR print the forms from the links below. (Please note that the GMS1 form will only be available from reception).

Step 3 - Fill in the forms and questionnaires.

Step 4 - Hand in the forms to reception along with two forms of identification for us to photocopy. One of the forms of ID will need to be photographic. For the registration of a child, we will accept a copy of the birth certificate or the red book


Registration Pack for Adults (16years+)

Inside the registration pack theres should be the following forms;

- GMS1 form (For registration - only available at reception)
- New Patient Questionnaire for Adults (16years+)
- Alcohol Intake Questionnaire
- Online Access Registration Form
- Consent for Sharing Form (For record sharing permissions)
- Opt out of Summary Care Records Form (If you do not wish to have a summary of your care available to other health and social workers who provide you care). Click here for more information on Summary Care Records.
- Third Party Access Consent Form (To give permission for someone other than yourself to book appointments, collect prescriptions and have access to your medical record)
