Third Party Access Consent

 

Due to protecting patient confidentiality and data sharing regulations, the surgery must have explicit consent from a patient to be able to release any information to a third party on behalf of the patient.

In order to give your consent to a third party please download and fill out the form below (or collect one from reception) and hand into the surgery where we will be able to make a note of the consent on your patient records.

 

There is no limit to the number of people that you give third party access to.

 

In the case of collecting prescriptions, the surgery does operate EPS (Electronic Prescription Service). This means that your prescription is sent electronically straight to your chosen pharmacy where you (or someone with your explicit permission) will then be able to pick up your medication without having to first visit the surgery. To nominate a surgery please download and fill out the form below (or collect one from reception) and hand into the surgery.

This service is designed to make it easier for the patient to collect their medication and is more environmentally friendly.
